Sec. 112.205. - Contents of notice.

In addition to the information required by F.S. § 768.28(6)(b), a notice served upon the General Counsel pursuant to this Chapter shall contain the following information: (a) The name and address of the claimant. (b) The date, time and place of the injury or occurrence complained of. (c) A description of the injury or damage suffered by the claimant. (d) The names of the agents or employees of the City or independent agency involved, if known to the claimant. (e) A statement, in general terms, of the relief sought from the City or independent agency. (Ord. 81-1047-479, § 2; Ord. 83-591-400, § 1) Note— Former § 120.105 .
