Sec. 382.304. - Filing of applications.

(a) An original and two copies of each application for a contract must be filed. An applicant may apply for no more than two service areas. (b) Applications shall be delivered to the Procurement Division no later than 5:00 p.m. on the date specified in the notice and, upon receipt, shall immediately be delivered by the Procurement Division to the Council Secretary with a copy to the Manager of the Office of Administrative Services or his designee. (c) Applications shall be sworn before a notary public or other officer authorized to administer oaths in the following terms: I swear (or affirm) under the penalties of perjury that the information given in this application is true and correct.
