Sec. 711.316. - Applications.

(a) Applications for a franchise or renewal thereof shall be submitted to the Director and shall include, but not be limited to, the following: (1) The name of the applicant, the address and telephone number of the applicant, and the applicant's Florida Sunshine State one-call registration number. (2) If applicable, a copy of the applicant's current certificate of authorization issued by the Florida Public Service Commission or the Federal Communications Commission, as applicable. (3) A certificate of insurance verifying workers' compensation, general liability and property damage insurance coverage or proof of self-insuring status which demonstrates adequate financial resources to defend and cover claims, in compliance with levels of coverage specified hereinafter. (4) Name of registered agent and incumbency certificate of authorized officer to bind the applicant. (5) The type of services (or other uses, water/sewer, etc.) that the facilities within the rights-of-way will be used to provide. (b) All grantees shall inform the City in writing of any change to the above information within 30 days following the date on which the grantee has knowledge of the change. (c) The City Council delegates to the designated Director the authority to administratively approve applications that comply with the requirements of this Part. (d) Standards for award or denial of application. The City Council, or where appropriate the designated Director, shall award a franchise (or requested franchise renewal) to an applicant where it is determined, based on the evidence in the record, that the applicant has complied with the requirements of this Chapter. Any denial by a Director shall, if requested by the applicant, be submitted by the Director to the City Council for its review of the record as considered by the Director. (Ord. 2001-427-E, § 2)
