(a)
Applications. Applications for permits required by this chapter shall be on forms provided by the official and shall contain the information required by the official.
(b)
Insurance. Proof of liability insurance, with the city as an additional insured, in the amount of one million dollars ($1,000,000.00), issued by an insurer licensed to do business in the state, is a prerequisite for the issuance of blasting and fireworks display permits. The official may require a higher dollar limit if he finds that, due to the location or nature of the permitted event, such higher limit would be in the city's or the public's interest. The official may require similar insurance for other events or activities requiring his approval when, due to the circumstances, nature or location of the event or activity, it is in the city's and the public's interest to do so.
