(1)

In addition to the requirements of part II of chapter 408, any person desiring to be licensed under this chapter shall apply to the agency and the application must contain such information as the department and the agency find necessary to determine the ability of the applicant to carry out its responsibilities under this chapter.

(2)

The applicant shall provide proof of liability insurance coverage in amounts set by the department and the agency by rule.
